Report Cybersecurity Incident

Service Description

This service allows faculty, staff, and students to formally report any suspected or confirmed cybersecurity incident, including:

  • Compromised Accounts: Suspected unauthorized access to user accounts, email, or other systems.
  • Malware or Virus Infections: Detection of malicious software on devices or network systems.
  • Phishing or Social Engineering Attempts: Receipt of suspicious emails, messages, or calls attempting to elicit sensitive information.
  • Unauthorized Access or Data Breach: Evidence of unauthorized access to systems, data, or networks.
  • Lost or Stolen Devices: Reporting the loss or theft of devices containing potentially sensitive information.
  • Other Suspicious Activities: Any unusual or concerning activity observed on network systems or devices. 
Information Needed For The Request

Please provide the critical information requested for efficient initial triage and response. This includes date and time of discovery, how the suspicious activity presented, any actions taken, what system(s) are affected, and whether any personal or sensitive data was shared.
